Seeking an experienced Nurse Practitioner to perform home health risk assessments in Crook County, OR, including Prineville and Ochoco West. This per diem role is ideal for providers interested in flexible scheduling and meaningful patient interaction, while supporting members of a Medicare Advantage health plan.

Key Responsibilities:

Complete annual and post-discharge home health risk assessments lasting approximately 45 minutes per visit Evaluate patients past and present medical history, conditions, and medication usage Educate individuals about their health conditions and prescribed medications Recognize gaps in care and make informed recommendations to address those needs No responsibility for prescribing medication or developing treatment plans

Qualifications:

Active Nurse Practitioner license in Oregon Board certification by AANP or

ANCC MSN

from an accredited nursing program Current CPR certification (BLS) At least 6 months of relevant NP experience preferred This position offers compensation of $80$90 per evaluation, along with mileage and additional administrative stipends.
