What they do
A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description
- Help clients navigate HIV treatment, nutrition, and overall health. Connect & Support
- Work closely with providers to ensure clients receive prompt access to services, helping to navigate their care journey with ease and support. Educate & Empower
- Provide clients with the knowledge and tools to understand their HIV treatment plans, harm reduction strategies, and empowering them to take control of their health and well-being. Monitor & Support
- Track lab results and attend appointments as needed. Advocate for Equity
- Provide culturally competent, client-centered care that empowers individuals to advocate for themselves, fostering independence and ensuring equitable access to all necessary resources. Collaborate
- Work with medical providers, laboratories, pharmacies, case managers, housing navigators, and community partners to ensure linkage to care and support services. What We're Looking For Oregon RN License

- Prior work with HIV care, harm reduction, or marginalized populations is highly preferred. Bilingual? Spanish-speaking candidates are strongly encouraged to apply. Valid Driver's License
