Conduct individual assessments and develop tailored care plans. Monitor vital signs, manage symptoms, and perform procedures like catheterization. Collaborate with interdisciplinary teams and educate patients/families on care options. Utilize EMR systems such as HomeCare Homebase for documentation. Address behavioral health concerns and support holistic patient well-being. Requirements include a valid Nurse Practitioner license, experience in outpatient or similar settings, proficiency with EMR, and a commitment to empathetic, patient-centered care.
