Key Responsibilities:

Conduct comprehensive annual or post-discharge home health risk assessments for Medicare Advantage members in their residences Assess patients current and past medical history, existing conditions, and medication lists Educate patients on their medical conditions and prescribed medications Identify gaps in care and provide actionable recommendations for care enhancement No requirement to prescribe medication or develop treatment plans

Qualifications:

Active Nurse Practitioner license in Oregon Current BLS CPR certification Board Certification through AANP or ANCC Minimum six months of recent Nurse Practitioner experience preferred MSN degree from an accredited nursing school

Compensation and Benefits:

Compensation for this per diem position is $80$90 per evaluation, plus a mileage stipend and compensation for administrative time.
