Job Description
At 365 Hospice , we believe in providing compassionate, patient-centered care to those we serve and their families. As a family-owned and operated hospice , we know that quality care starts with a supportive, dedicated team. We are currently seeking an On-Call Registered Nurse to cover weekend shifts and provide exceptional end-of-life care. This on-call position would be from 4:30pm on Friday until 8:00am the following monday morning. This would be a weekend position and be salaried at full time. The on-call position is only required to leave your house when called out Key Responsibilities Provide skilled nursing assessments and care for hospice patients in their homes or facilities. Respond promptly to urgent calls and make home visits as needed. Collaborate with the hospice team to ensure patients and families receive comprehensive support. Educate patients, families, and caregivers on hospice philosophy and plan of care. Document all care provided in compliance with hospice standards and regulations. Completes an initial assessment of patient and family to determine hospice needs. What We're Looking For Current Registered Nurse (RN) license in Pennsylvania. Minimum 1 year of clinical nursing experience (hospice or home health preferred). Strong critical thinking, communication, and compassionate care skills.
