Core Responsibilities:

Deliver in-home nursing care, including skilled interventions and education Perform patient assessments and ensure OASIS documentation is timely and accurate Develop and implement individualized care plans in collaboration with physicians and care teams Provide wound care, medication management, chronic disease monitoring, and teaching Keep the clinical team informed of patient status and significant changes

Preferred Qualifications:

Active RN license At least 1 year of recent home health RN experience Strong OASIS documentation skills required Familiarity with HomeCare HomeBase strongly preferred Ability to organize and manage a full schedule of daily home visits Valid driver's license and reliable transportation
