Job Description

Registered Nurse (RN) - Home Health - Per Visit / PRN Penn Home Health Care is a Medicare-certified home health agency seeking experienced and compassionate Registered Nurses (RNs) to join our skilled home health team. We are looking for clinicians who are committed to providing high-quality patient care while helping patients remain safely in their homes.
Competitive Per-Visit Rates SOC:
$170 (+$30 incentive for same-day documentation completion)

Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, Pennsylvania Position Type:
Per Visit /
PRN Responsibilities:
Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Supervisory, Routine Skilled Nursing, and Discharge visits. Perform comprehensive patient assessments. Develop and implement individualized plans of care in collaboration with physicians. Provide skilled nursing services including disease management, medication management, wound care, patient education, and care coordination. Complete OASIS assessments as assigned. Communicate significant findings and changes in condition to physicians and interdisciplinary team members. Participate in case conferences and quality improvement activities. Maintain accurate and timely electronic documentation.
Qualifications:
Current Pennsylvania Registered Nurse license in good standing. Current CPR certification. Valid driver's license and reliable transportation. Minimum one year nursing experience preferred. Home health experience preferred. OASIS experience preferred but willing to train qualified candidates.
