YOUR ROLE

As a Hospice RN Case Manager, you'll be a trusted clinical presence for your patients and their families. You'll coordinate care, manage symptoms, provide education and support, and collaborate with physicians, caregivers, and the hospice interdisciplinary team. Responsibilities Manage and coordinate individualized hospice patient care plans Perform comprehensive nursing assessments and ongoing patient evaluations Provide expert symptom management and compassionate nursing care Educate patients and families about disease progression, medications, and comfort-focused care Collaborate with physicians, caregivers, and interdisciplinary team members Maintain accurate, timely, and thorough clinical documentation Provide emotional support and guidance to patients and loved ones Advocate for patient comfort, dignity, and quality of life

WHAT WE'RE LOOKING FOR

We're seeking an RN who combines strong clinical skills with compassion, professionalism, and a genuine commitment to patient-centered care. Qualifications Active Pennsylvania RN license Strong clinical assessment and organizational skills Excellent communication and interpersonal abilities Compassionate, patient-centered approach Ability to work independently and as part of a team Valid driver's license and reliable transportation
