Full Job Description Role Overview:

The RN Nurse Navigator Triage provides nursing triage services and health advice via phone and in person, assessing patient acuity and determining medical support needs. They deliver results, education, and protocol-driven clinical advice to ensure quality, continuous care.

Key Responsibilities:

Conduct patient assessments, manage urgent situations, prioritize high-priority calls, coordinate care across departments, and assist with patient scheduling and communication. They also support new staff training, explain medical conditions, and facilitate external care coordination with organizations like insurance and home health. Ensuring timely documentation and patient satisfaction are critical.

Qualifications & Skills:

A graduate of a nursing specialty training program is required, with a BSN preferred. Five years of nursing experience, especially in ICU, ED, or Critical Care, is strongly preferred. Proficiency in computer skills and Microsoft Office Suite is essential. Knowledge of clinical protocols, strong communication, and coordination skills are necessary.
