Full Job Description Role Overview:

Provides patient education, promotes understanding of conditions, and ensures ongoing continuity of care. Acts as a patient navigator, coordinating pre- and post-hospital care, facilitating patient entry into care programs, and serving as a liaison between patients and healthcare teams.

Key Responsibilities:

Coordinate multidisciplinary clinics and care activities. Communicate with patients via mail, phone, and in person to support their care plans. Assist in diagnosis and treatment support, collaborating with healthcare providers. Guide patients through care processes, reinforce education, and connect to support services. Maintain follow-up systems to monitor post-care patients. Provide nursing support and assist physicians during procedures. Educate team members and ensure compliance with policies.

Qualifications & Skills:

Graduate from a Specialty Training Program in Nursing (required). Bachelor's Degree in Nursing preferred. At least 2 years of RN experience required. Strong patient communication, coordination, and clinical skills. Ability to work in a clinical environment and collaborate effectively.
