Registered Nurse, RN - Hospice Admissions/ Community Liaison Monday thru Friday 8:30am to 5:00pm

To learn more about Traditional Home Health Care Job responsibilities include, but not limited to the following: Perform start of care visits on hospice patients and admit patients to Traditional Home Health Care, completing clinical documentation as required Initial evaluations for hospice appropriateness Obtaining consents during initial hospice discussion with patients and families Assesses patient status and develops a plan of care with coordination of nursing supervisor, physician, patient and family members Coordination of hospice referrals including obtaining records/orders/diagnosis Maintenance of electronic hospice contracts Assist with transition of care between palliative program and hospice program Hospice philosophy teaching Coordination of care audits with our

SNFs Requirements:

Current license as a Registered Nurse in Pennsylvania Excellent assessment skills, documentation skills and ability to meet deadlines Must have a current drivers license Ability to travel to case assignments Ability to work independently with minimal supervision and manage time effectively Strong interpersonal skills Demonstrates good verbal and written communication, and organizational skills Acute care and/or home health experience is preferred Why Choose Traditional Home Health?
