Hospice RN - Wyoming Valley, PA $3,000 Sign-On Bonus Serenity Hospice is seeking a compassionate and dedicated Registered Nurse (RN) case manager to join our growing team serving the Hazleton/Weatherly, Pennsylvania area. This is a rewarding opportunity to provide meaningful care and support to patients and families during an important time in their lives.

Responsibilities:
Provide skilled nursing care to hospice patients in their homes or care facilities Perform patient assessments and develop individualized care plans Collaborate with the interdisciplinary hospice team Educate patients and families on disease progression and comfort care Maintain accurate and timely clinical documentation
Qualifications:
