Job Description

Job Overview We are seeking a dedicated and compassionate Weight Loss Nurse to join our dynamic healthcare team. In this vital role, you will provide specialized nursing care focused on supporting patients through their weight loss journeys. Your expertise will help guide, educate, and motivate individuals striving to achieve healthier lifestyles. This position offers an exciting opportunity to make a meaningful impact on patients' lives while working within a collaborative, patient-centered environment. The ideal candidate will bring a strong background in various clinical settings, including hospital medicine, outpatient clinics, and post-acute care, with a focus on patient assessment and personalized care plans. Responsibilities Conduct comprehensive patient assessments to determine suitability for weight loss programs and identify any contraindications or health risks Develop and implement individualized weight management plans in collaboration with physicians and multidisciplinary teams Monitor patient progress through vital signs, physiological data, and diagnostic evaluations, adjusting care plans as needed Educate patients on nutrition, exercise, medication management, and behavioral health strategies to support weight loss goals Manage discharge planning and follow-up care to ensure continuity of support after initial treatment phases Maintain accurate medical records in compliance with HIPAA regulations and hospital policies Assist with intake procedures, medical-surgical assessments, and case management activities related to weight loss treatment programs Skills Critical care experience with a strong understanding of physiology and acute care principles Proficiency in medical terminology, diagnostic evaluation, discharge planning, and utilization management Skilled in patient service delivery with excellent patient assessment capabilities and attention to detail Familiarity with medical records management and post-acute care processes including outpatient clinic experience Ability to work effectively within managed care environments while maintaining high standards of patient care Join our team to empower patients on their journey toward healthier living!
