In this role, you will conduct comprehensive evaluations, coordinate care plans, and collaborate with multidisciplinary teams to improve outcomes and reduce healthcare costs. Responsibilities Perform comprehensive in-home and community-based health assessments, including history, physical exams, and preventive screenings. Identify, evaluate, and help manage acute and chronic medical conditions within the Nurse Practitioner scope of practice. Develop and update individualized care plans focused on improving outcomes, safety, and quality of life for each patient. Review and reconcile medications, identify potential interactions or adherence issues, and provide medication counseling. Educate patients and families on disease management, lifestyle modifications, and appropriate use of healthcare resources. Coordinate care with primary care providers, specialists, care managers, and health plan partners, communicating key findings and recommendations. Document all encounters accurately and timely in electronic health record systems, ensuring compliance with regulatory and quality standards. Participate in quality assurance and performance improvement initiatives, following evidence-based guidelines and company protocols. Work independently in the field, traveling within an assigned territory and managing a schedule that meets productivity and quality expectations. Adhere to all safety, infection control, and privacy regulations while providing care in patients' homes and community settings.
