Key Responsibilities:

Perform comprehensive annual home health risk assessments, each averaging 45 minutes per visit Review prior and current medical histories, chronic conditions, and medication lists with patients Deliver patient education regarding ongoing health issues and medications as needed Identify care gaps and provide focused recommendations to improve health outcomes No treatment planning or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Pennsylvania Board Certification through either AANP or

ANCC MSN

degree from an accredited nursing school Current certification in CPR (BLS) Minimum of 6 months of recent Nurse Practitioner experience preferred
