to offer comfort, dignity, and support when it matters most. We're seeking a Registered Nurse Case Manager who is passionate about making a difference. As a key part of our interdisciplinary team, you'll oversee and coordinate the care of hospice patients, ensuring they receive the highest level of clinical and emotional support. What You'll Do Manage a caseload of hospice patients, providing comprehensive nursing care Develop and implement individualized care plans Communicate closely with patients, families, and the hospice team to ensure comfort and understanding Provide education and emotional support to families during end-of-life care Ensure documentation meets compliance and quality standards What We're Looking For Active RN license in Pennsylvania Hospice or home health experience preferred (but we'll train the right nurse!)
