Job Description

Embark on a rewarding healthcare role providing essential nursing care to patients in a field-based home health setting. This full-time, 13-week contract offers an engaging Monday through Friday schedule, with shifts from 8:00 AM to 5:00 PM, and an expected caseload of 30 points per weekgenerally translating to 35 patient visits per day. Youll deliver skilled nursing services directly in patient homes, supported by industry-leading electronic documentation using Home Care Home Base (HCHB) and the OASIS assessment tool.
Qualifications:
Active RN license in good standing for the state of practice At least one year of recent experience in Home Health (field experience highly preferred) Proficient with Home Care Home Base (HCHB) documentation system OASIS assessment experience required Reliable transportation, valid drivers license, and current auto insurance
Key Responsibilities:
Complete comprehensive OASIS assessments and develop individualized care plans Provide skilled nursing carewound care, IV therapy, medication administration, and chronic disease management Monitor patient progress, communicate changes to the care team and physicians Educate patients and families regarding treatment, medications, and preventive health Accurately maintain patient documentation using HCHB Collaborate with the broader care team (PT, OT, medical social workers, aides) for holistic patient care Participate in team conferences, meetings, and continuing education Uphold agency and regulatory standards for care delivery and documentation Benefits include weekly pay, access to medical, dental, and vision insurance, 401(k) retirement planning, round-the-clock support, mileage reimbursement, and the possibility of contract extensions.
