Three-month contract, possibility of an extension

Schedule:

Monday-Friday, 8:00 AM - 4:30

PM On-Call Requirement:

One to two nights per week, and one weekend of on-call coverage per month

Pay Rate:

$53.00 - $55.90 per hour, mileage reimbursement Responsibilities Provide skilled nursing services in patients' homes throughout the assigned coverage area Manage patient caseloads and coordinate ongoing care Complete comprehensive patient assessments and documentation Develop, implement, and monitor individualized care plans Collaborate with physicians and interdisciplinary care teams Educate patients and caregivers on treatment plans and disease management Ensure accurate and timely documentation of all patient care activities Requirements Active Pennsylvania Registered Nurse (RN) license Home Health experience required Strong case management experience Ability to work independently in the field Reliable transportation for local travel Strong assessment and documentation skills Preferred Skills Experience with HCHB (Homecare Homebase) documentation Strong care coordination and case management skills Excellent communication and organizational abilities Experience managing a home-based patient population

Pay Details:
