PM Key Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized patient care plans. Deliver skilled nursing care, such as wound management, medication administration, IV therapy, and chronic disease oversight. Evaluate patient progress, recognize and communicate changes in condition, and coordinate with other healthcare professionals. Educate patients and families regarding disease management, treatment protocols, and preventive strategies. Complete timely and thorough documentation using the Home Care Home Base (HCHB) electronic system. Work collaboratively with physical and occupational therapists, medical social workers, and home health aides for coordinated care delivery. Participate in regular case conferences and staff meetings, ensuring compliance with regulatory standards and agency expectations. Travel daily to patient homes within your assigned territory; mileage reimbursement is provided.

Qualifications:

Active, unencumbered RN license in the state of practice. At least one year of recent home health nursing experience, with field experience strongly preferred. Proficient with Home Care Home Base (HCHB) and OASIS assessments. Reliable transportation, valid drivers license, and current auto insurance are essential.
