In-Person Competitive Salary 3 Years Experience Vocational/Technical Certificate No Commission 40.00 hours per week / Day, Evening, Weekend Shift / Full-Time Description The Registered Nurse, Community Living (Nurse) provides clinical care and case management to approximately 15-20 individuals with medical fragility, Intellectual and Developmental Disabilities (IDD) and Mental Health (MH) diagnoses. The individuals served reside in group homes regulated by the state of Pennsylvania under the 6400 regulations. The Nurse utilizes both clinical and case management skills to ensure individuals receive appropriate and high-quality care. Clinical skills demonstrated on the job may range from tasks including, but not limited to, capturing weights, blood pressure, assessing wounds and catheter care. Case management tasks include, but are not limited to, scheduling and following up on medical appointments, ensuring new medication orders are received and ordering durable medical equipment. Additionally, the Nurse coaches and trains direct support professionals (DSPs) in the home specifically as it pertains to recognizing medical emergencies and on how to provide daily care to the residents to ensure positive health outcomes.
DUTIES AND RESPONSIBILITIES
Complete clinical tasks including, but not limited to, wound care, operating a feeding pump, administering supplemental oxygen, assessing fall risks, completing lung and abdominal exams, doing post ER and hospitalization assessments which include a skin and vitals check, lung assessment, and mental status; troubleshoot catheter, feeding pump, or issues with adaptive durable medical equipment to ensure positive health outcomes for residents. Coach and train staff on skills such as, but not limited to, how to give bed baths, repositioning and taking vitals; identify, advise, and train staff on illnesses and signs to look for that may indicate an imminent emergency such as a urinary tract infection (UTI), bowel obstruction, or stroke; train and coach staff on how to respond to 911 emergencies such as allergic reactions, diabetic shock, cardiac, or breathing difficulties to ensure the safety and care of all in the home. Communicate and collaborate with external therapists and other related services providers such as contracted nurses, occupational therapists (OT), physical therapists (PT), and Speech (SLP) to ensure continuity of care; ensure prescribed treatment plans from external clinicians are being followed with the residents on their caseload to ensure the health and safety of individuals. Seek expert education, and train others in proper use of all prescribed adaptive equipment for individuals served to ensure proper use always. Understand the nutritional needs and meal texture required for the individuals and train and coach staff on how to provide nutritious meals in the correct texture to ensure the safety and health of individuals in their care. As the primary contact for external physicians and specialists, complete all paperwork for appointments and include all necessary documents related to tests, treatments, etc. for the physician (i.e. ensuring x-rays or prior labs needing review during the appointment are available). Complete documentation in the electronic health record including, but not limited to, the lifetime medical history, case notes, assessments, appointments, etc. and ensure documentation is compliant with 6400 regulations. Utilize the established pharmacy system to order new medications or refills and keep track of and communicate when medication changes have occurred and when the medications have arrived at the homes to ensure individuals are getting the correct medication and staff are aware of any changes to medications. Support staff in medication administration record (MAR) documentation per 6400 regulations, review MAR for accuracy, and observe staff giving medications to ensure correct administration and provide feedback. Audit medications, ensuring the texture is correct and the administration method is accurate (whether measuring liquid, crushing pills, or giving in specific foods) to minimize error and ensure appropriate dosing and delivery. Communicate with medical decision makers and next of kin and document correspondence in the electronic health record to ensure an accurate record of information shared. Coordinate care during ER visits and hospitalizations, communicate with hospital social workers, update electronic health record to include new orders post hospitalization and ensure all post hospitalization orders are followed as prescribed. Participate in team meetings as required and attend medical appointments as needed. Keep electronic and paper charts up to date with the most current information so staff may have correct information regarding each individual to ensure their health and safety. Participate in rotating on-call duties as assigned. Perform other duties as assigned. Education/Experience/Skills Requirements Active, valid licensure as a Registered Nurse (RN) in the Commonwealth of Pennsylvania. Prior experience with Community Living Arrangements and case management. Three (3) years' experience working with individuals living with multiple disabilities, including a strong understanding of the safety and communication support required for individuals with physical disabilities Demonstrated excellent judgment with the ability to independently solve problem and make decisions with little direct supervision Demonstrated experience working with feeding tubes, colostomies, tracheotomies, catheter, and wound care strongly preferred. Demonstrated ability to work in a fast-paced environment and effectively prioritize tasks Demonstrated strong attention to detail Demonstrated ability to work effectively as part of a team Demonstrated strong time management and organizational skills Demonstrated ability to anticipate changes and/or identify issues and take appropriate and effective actions Excellent customer interaction, collaboration, and written and verbal communication skills Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, SharePoint and PowerPoint Experience using electronic management/electronic records systems Must be able to perform CPR, First Aid, and crisis intervention using agency-trained protocols Ability to attend meetings off site as necessary Ability to work nights and weekends as needed Additional Details How To Identify Potential Job Scams
