Monday Friday, 8:00 AM 5:00

PM Role Highlights:

Manage a caseload of approximately 30 points per week (average of 35 patient visits daily) Provide care directly in patient homes within an assigned area, with mileage reimbursement for travel Utilize Home Care Home Base (HCHB) for all documentation Conduct detailed OASIS assessments and formulate personalized plans of care

Key Responsibilities:

Complete comprehensive OASIS assessments to guide individualized patient care Deliver skilled nursing services, including wound care, IV therapy, medication administration, and management of chronic illnesses Monitor and assess patient progress, communicating changes to the interdisciplinary team and physicians Educate patients and families regarding conditions, care plans, and preventative health strategies Maintain thorough, timely documentation utilizing HCHB Collaborate with therapy staff, social workers, and home health aides to ensure seamless care Attend case conferences, staff meetings, and participate in ongoing training Uphold regulatory and organizational best practices for safe and effective care delivery

What Were Looking For:

Current, active RN license in good standing (state of assignment) At least one year of recent field home health nursing experience Proficiency with Home Care Home Base (HCHB) and OASIS assessments Dependable transportation, a valid drivers license, and current auto insurance
