Responsibilities:

Conduct annual and post-discharge home health risk assessments, each lasting approximately 45 minutes Review and document prior and current medical histories, chronic medical conditions, and medication lists Educate members on their medical conditions and medication regimens Identify care gaps and recommend appropriate actions to address them No requirement to prescribe, treat, or develop care plans

Qualifications:

Active Nurse Practitioner license in Pennsylvania Valid CPR certification (BLS) Certification through AANP or ANCC Minimum of six months recent NP experience preferred Master of Science in Nursing (MSN) degree from an accredited institution Enjoy competitive compensation of $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
