Job Description

Description Affinity is looking for reliable, compassionate, and professional Registered Nurses (RNs) to care for our patients in the West Philadelphia area. The ideal RN professional is highly skilled, organized, and dedicated to providing quality patient care. We have a variety of opportunities available including Shift Care, Per Diem, Start of Care (SOC), Skilled Visits, and Hourly Assignments based on your availability and preferences. We use Axxess and HHAexchange. Responsibilities Record and assess a patient's medical history accurately Monitor and record vital signs including blood pressure, temperature, heart rate, and oxygen saturation Perform Start of Care (SOC) visits and ongoing patient evaluations Develop and update patient care plans in coordination with physicians Monitor patients' conditions and identify progress or side effects of treatments Administer medications, treatments, and IV therapy as prescribed Educate patients and families while providing emotional support Maintain accurate documentation and ensure compliance with home health standards Communicate effectively with physicians, caregivers, and interdisciplinary team members Complete documentation and visit notes in a timely manner Skills Proven experience as a Registered Nurse Ability to manage patient care independently and within a team Familiarity with home health care standards and procedures Strong understanding of confidentiality and nursing best practices Excellent knowledge of medical terminology Experience with electronic charting systems Strong clinical assessment and organizational skills Excellent communication and interpersonal skills Compassionate and able to handle stressful situations professionally Home Health experience preferred Active Pennsylvania RN License required Reliable transportation required Apply through the app or send your resume to: ssaenz@affinityhome.net About Affinity HealthCare Solutions Skilled Care LLC Affinity Healthcare Solutions Skilled Care, LLC is a CHAP accredited, Medicare Certified home health agency.
