Schedule:
Monday-Friday Full-Time Day shift only No weekends Fixed schedule/no on-call
Key Responsibilities:
Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed); Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists; Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment; Develops and maintains a plan of care that describes interventions to attain expected outcomes; Implements and records interventions to attain expected outcomes in EMR; Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections; Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.); Maintains a positive and respectful attitude to all patients and colleagues; Occasional travel to other nearby locations; and Medication administration and documentation.
Requirements:
Valid RN license BS/AA/diploma in nursing 6 months of RN experience strongly preferred 3 months of hemodialysis experience strongly preferred Valid drivers license and transportation Ability to travel covering multiple locations BLS certification (American Red Cross or American Heart Association Required); and Ability to lift up to 35 pounds is required This is a safety sensitive position
