Job Summary We are seeking a dynamic and compassionate Advanced Practice Nurse (APN) to join our healthcare team. In this vital role, you will provide expert patient care, perform comprehensive assessments, and collaborate closely with Mental Health Professionals & Registered Dietitians. Your expertise will help deliver high-quality, patient-centered care across diverse clinical settings, including an (IOP) intensive outpatient program to come and on an outpatient basis until our IOP program starts. We work with children, adolescents, adults and families which include a variety of mental health diagnosis, and we specialize in eating disorders, body image etc. This position offers an exciting opportunity to utilize your advanced nursing skills to make a meaningful difference in patients' lives while working within a collaborative team in a creative environment. Duties Conduct thorough physical examinations and patient assessments to determine health status and develop individualized care plans. Prescribe medications, including controlled substances in accordance with DEA regulations, and manage medication administration. Perform diagnostic evaluations, medication prescribing and management, taking weights and vitals of clients as needed. Collaborate with healthcare professionals across disciplines to coordinate patient care, discharge planning, case management, and utilization review. Manage electronic health records (EHR/EHR systems like Therapy Notes), ensuring accurate documentation of medical records, coding (ICD-10/ICD-9), CPT coding, and medical documentation standards. Provide patient education on health coaching & medication management Skills Extensive experience working with diverse patient populations including pediatrics, behavioral health, and individuals with disabilities or developmental disabilities. Knowledge of healthcare laws including HIPAA compliance and Workers' Compensation law; understanding of CPT coding for billing purposes. Experience in telehealth services delivering remote patient assessments and follow-up care effectively. Ability to work collaboratively within multidisciplinary teams to optimize patient outcomes while demonstrating excellent client communication skills.
