25-50% within ~50-mile radius (mileage reimbursed) Overview Join a leading integrated care team supporting dual-eligible (Medicare/Medicaid) members with complex medical and social needs. This is a work-from-home role with local travel to meet members face-to-face and coordinate care across the continuum. Key Responsibilities Perform comprehensive member assessments using clinical tools, claims/data review, and multiple information sources Develop and manage proactive care plans to improve short- and long-term outcomes Identify and address risk factors, barriers, and social determinants of health impacting care Coordinate referrals to appropriate clinical resources and interdisciplinary team members Present cases at interdisciplinary case conferences as needed Use motivational interviewing to drive engagement and support behavior change Maintain documentation and processes in compliance with regulatory and company guidelines Required Skills & Qualifications Active, unrestricted RN license in New Jersey 3+ years clinical practice experience 2+ years in Care Management, Discharge Planning, and/or Home Health Care Coordination Willing/able to travel 25-50% using personal vehicle Proficiency with computers and navigating multiple systems (
