A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.
Job Summary We are looking for a compassionate and motivated LPN or RN to join our growing Care Management Program. This is a nearly fully remote nursing position designed for someone who wants to make a meaningful impact on patients' lives while stepping away from the traditional bedside or clinic setting. The vast majority of your work will be completed remotely, with minimal in-person work in Sandpoint and the surrounding area as needed. You'll build ongoing relationships with patients, becoming a consistent and trusted part of their healthcare journey. Rather than only interacting with patients when they're sick, you'll help them stay healthier between visits by checking in, monitoring their progress, identifying concerns early, and helping them navigate their care. What You'll Do Build meaningful, ongoing relationships with patients through phone and virtual check-ins Support patients participating in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Advanced Primary Care Management (APCM) Help patients understand their health conditions, medications, and care plans Develop individualized care plans and help patients work toward achievable health goals Review remote monitoring readings and identify potential concerns early Coordinate with providers and clinic teams when additional support is needed Help patients overcome barriers to care and connect them with helpful resources Document patient interactions, progress, and care management activities Serve as a consistent connection between patients and their healthcare team Qualifications Active, unrestricted LPN or RN license required Care Management Certification preferred Strong communication and relationship-building skills Compassionate, dependable, organized, and patient-focused Comfortable working independently in a remote environment Comfortable learning and using healthcare technology Previous care management, primary care, chronic disease management, CCM, RPM, or population health experience is a plus Ability to occasionally travel within Sandpoint and surrounding areas when needed Why This Work Matters Healthcare doesn't stop when a patient leaves the doctor's office—and neither should their support.
