POSITION SUMMARY SU
Caring Hands Home Healthcare LLC is seeking an experienced, dependable, and highly organized Director of Nursing to oversee the agency's clinical operations. The ideal candidate will have strong Medicare-certified home health experience, proven clinical leadership skills, and working knowledge of CMS Conditions of Participation, OASIS, PDGM, QAPI, clinical documentation, and regulatory survey readiness. This position will begin on a part-time basis while the agency grows, with the opportunity to increase hours and transition to full-time as the patient census increases.
KEY RESPONSIBILITIES
Provide leadership, oversight, and direction for all clinical services Ensure compliance with CMS Conditions of Participation, Ohio regulations, ACHC standards, and agency policies Supervise and support RNs, LPNs, home health aides, therapists, and other clinical personnel Review referrals and assist with clinical admission decisions Review plans of care, physician orders, clinical documentation, recertifications, transfers, and discharges Monitor OASIS accuracy, completion, timely submission, and documentation compliance Participate in QAPI activities, clinical record audits, infection-control monitoring, and performance-improvement initiatives Coordinate patient care with physicians, therapists, patients, families, and other healthcare professionals Monitor completed visits, missed visits, hospitalizations, complaints, incidents, and changes in patient condition Assist with staff recruitment, orientation, competency evaluations, education, supervision, and annual performance reviews Conduct supervisory and field visits when required Maintain continuous survey readiness and assist with plans of correction when necessary Participate in on-call clinical coverage as assigned Promote high-quality patient care and a positive, professional, and accountable workplace culture
REQUIRED QUALIFICATIONS
Current and unrestricted Ohio Registered Nurse license Minimum of three years of recent skilled home health nursing experience Previous clinical leadership or supervisory experience in a Medicare-certified home health agency Strong knowledge of CMS home health requirements, OASIS, PDGM, QAPI, plans of care, and clinical documentation Knowledge of ACHC or another recognized home health accreditation organization Strong leadership, organizational, communication, and problem-solving skills Ability to supervise clinical personnel and manage multiple priorities Ability to use electronic medical records and other healthcare technology systems Valid driver's license, reliable transportation, and current automobile insurance Ability to successfully complete all required background checks and employment screenings
PREFERRED QUALIFICATIONS
Three to five years of Medicare-certified home health experience Previous experience as a Director of Nursing, Clinical Manager, Nursing Supervisor, or Patient Care Manager Experience preparing a home health agency for accreditation or regulatory surveys Experience with QAPI, infection control, clinical record audits, staff education, and performance improvement Bachelor of Science in Nursing preferred but not required
SCHEDULE AND WORK ARRANGEMENT
This position will remain part-time while the agency grows, with the opportunity to transition to full-time as the patient census increases.
Expected hours:
20-30 hours per week Hybrid work arrangement In-office work required Monday through Wednesday Remote work permitted Thursday and Friday Field and supervisory visits throughout Central Ohio may be required Occasional on-call or after-hours clinical support may be required The schedule and work location may be adjusted based on patient care and operational needs
COMPENSATION
$40•50 per hour Compensation will be based on the candidate's qualifications, Medicare home health experience, and clinical leadership experience Opportunity for additional hours and transition to full-time as the agency grows
