A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Job Description

QUALIFICATIONS
Required :
Current Oregon and CA RN License; BLS and ACLS certification by end of month from 60 days of hire and maintained. Willing to work varied hours with flexible schedules. Excellent communication skills both orally and written.
Preferred :
BSN, Minimum 2 years' experience in Cardiology, Intensive Care or Heart Failure.
Lic/Reg/Cert :
Current Oregon and California RN license, willingness to obtain CHFN certification.
TYPICAL PHYSICAL/MENTAL DEMANDS
Full-time, Monday through Friday. Work is primarily in a clinical setting, with both inpatient and outpatient responsibilities. Includes inpatient and outpatient consultations, patient and family education, and ongoing remote monitoring of patient weights and blood pressures. Responsible for maintaining communication between patients and providers to ensure continuity of care. Works closely with pharmacy for medication titration and consultation. Some remote or telehealth follow-up may be required. Must be able to work collaboratively in a fast-paced, multidisciplinary environment. Standing for extended periods may also be required.
ESSENTIAL JOB FUNCTIONS
Serve as the primary point of contact for patients with heart failure, helping them navigate their care and treatment plan.
  • Provide education and counseling on heart failure self-management, including lifestyle changes, medication adherence, and symptom monitoring.
  • Conduct patient assessments and monitor progress, adjusting care plans as necessary.
  • Document all interactions, assessments, and interventions in the electronic health record (EHR).
  • Track patient outcomes and provide data for quality improvement initiatives.
  • Facilitate communication between patients, families, providers, and the heart failure care team.
  • Participate in case reviews, care planning meetings, and other team-based activities.
  • Manage inventory of clinical supplies and assist with billing and documentation related to patient care.
