Homecare RN- Hybrid Schedule - $2,500 Hiring Bonus Seeking a full-time Homecare RN nurse to work within ESWA's Nursing Department. The role of the Homecare RN is to assess and educate older adults for home services to help them age in their own home safely, independently and comfortably. The Homecare RN functions independently and as part of an interdisciplinary team. Nurses assess older adults for level of care needs (personal care vs home health aide), home health services (medication prefills, OT eval for safety equipment, home based footcare). In addition, nurses may complete screening activities to determine if an older adult meets clinical eligibility requirements for programs such as a frail elder waiver, adult day health program or an enhanced care options (ECOP) program. The nurse is responsible for maintaining appropriate paper and electronic files in accordance with Executive Office of Aging and Independence (AGE) and MassHealth regulations and standards.

QUALIFICATIONS/ABILITIES

1. A valid Massachusetts license to practice as a Registered Nurse required . 2. At least one (1) year of clinical experience and/or nursing in a community or long-term care setting 3. Use of a reliable vehicle including active vehicle insurance with a current license to drive required . 4. Computer proficiency, including experience with Microsoft products (Excel, SharePoint, Word, Outlook) 5. Demonstrate strong organizational, prioritization, and problem-solving skills. 6. Ability to multi-task, keep accurate records, have a strong attention to detail, document clearly, and communicate effectively verbally and in writing. 7. Ability to take initiative and work independently. 8. Must have moderate physical mobility. Ability to lift and carry objects weighing up to 20 pounds. Ability to climb stairs. 9. Ability to work in an interdisciplinary model.

• A staged hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year of active employment. The bonus for this position is $2,500.
