Hospice Intake Nurse (LPN) - Hybrid after training Foundations Hospice - Albany, LA (near Hammond) Winner of SHP's 2025 Best Hospice Caregiver Experience award. Foundations is a family-owned, Medicare-certified, CHAP-accredited hospice, and we're looking for a detail-oriented LPN to be the first clinical voice our patients and referral partners hear. About the role As our Intake Nurse, you'll be the hub between our marketers, clinical team, and Medical Director. You'll make sure every referral gets a timely, accurate review so patients get the right care when they need it. After on-site training at our Albany office, this position is hybrid : part in the office, part from home.
