Hospice Intake Nurse (LPN) - Hybrid after training Foundations Hospice - Albany, LA (near Hammond) Winner of SHP's 2025 Best Hospice Caregiver Experience award. Foundations is a family-owned, Medicare-certified, CHAP-accredited hospice, and we're looking for a detail-oriented LPN to be the first clinical voice our patients and referral partners hear. About the role As our Intake Nurse, you'll be the hub between our marketers, clinical team, and Medical Director. You'll make sure every referral gets a timely, accurate review so patients get the right care when they need it. After on-site training at our Albany office, this position is hybrid : part in the office, part from home. What you'll do Receive and process referrals from our marketing team Review patient assessments and clinical records Evaluate patients for hospice eligibility and appropriateness of care Manage and maintain accurate clinical documentation Communicate with nurses, social workers, marketers, and our Medical Director Take occasional on-call shifts on a rotating basis, with extra pay Schedule Rotating shifts: 8:30 AM-4:30 PM and 10:00 AM-6:00 PM Some on-call required What we offer Hybrid work from home after training Competitive pay plus additional on-call pay 100% company-paid $50,000 life insurance Health (we pay 75% for employee) and 100% dental, and vision insurance Generous PTO, with our top tier (160 hrs/year) reached at 5 years instead of the usual 10 Voluntary short- and long-term disability and supplemental life insurance A supportive, family-owned culture where leadership knows your name What you'll need Active Louisiana LPN license Strong attention to detail.
