Nursing Full Job Description Position Summary :

The Life Sharing Nurse is a key member of our Life Sharing team and reports to the Residential Manager. This role is responsible for supporting the health and wellness of both adults and children with intellectual disabilities and autism who are in life sharinghomes, including those with more complex medical needs (MCC). You will assist the Life Sharing Specialist in completing health assessments (HRST), and work to help organize and monitor medical services. A person-centered approach is essential, and the nurse will work collaboratively with program staff and families to ensure safe and supportive care. This position includes travel to people's homes for visits, with flexibility to telework from home or the office when not in the field.

Key Responsibilities:

Complete and maintain Health Risk Screening Tool (HRST) for people supported Monitor health, wellness, and medical needs in life sharing homes Support coordination of medical appointments and services Provide training to caregivers and staff on health-related topics Ensure accurate medical documentation and timely updates Support the safe administration of medications and review medication errors as needed Collaborate with the Life Sharing Medical Coordinator to improve care and address health gaps Participate in team meetings, quality improvement activities, and strategic planning Strategic Leadership & Collaboration Promote and model the mission and values of KZL Agency in all work Use HRST and other tools to help track and improve health outcomes Participate in planning and development of wellness strategies Help develop internal training and ensure quality standards are met Assist with quality management initiatives and reporting when needed

Qualifications:

Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current PA license• Minimum of 4 years nursing experience Experience with Intellectual Disabilities, Autism, or Mental Health preferred Experience handling emergency or acute medical situations preferred Valid PA driver's license and access to a personal vehicle Must pass Act 33 / 34 and FBI clearances (working with children) Excellent personal and professional references Why Join KZL Agency?
