This is a full-time hybrid role based in Canton, MI, offering flexibility for remote work. In this role, you will guide patients through the healthcare system, proactively address potential medical concerns, and coordinate personalized care plans for individuals with chronic and complex conditions. You'll play a critical role in improving outcomes while building meaningful patient relationships. Key Responsibilities Navigate and coordinate care across healthcare systems Identify and prevent potential medical complications Develop and implement individualized care plans Support patients with chronic and complex conditions Collaborate with providers, families, and interdisciplinary care teams Maintain accurate documentation and ensure compliance with healthcare regulations Registered Nurse (RN) Requirements Active and unrestricted RN license in the state of Michigan Minimum of 1-2 years of clinical experience (case management, home health, rehab, or acute care preferred) Strong understanding of care coordination, discharge planning, and chronic disease management Knowledge of healthcare regulations, documentation standards, and compliance requirements Experience with EMR/EHR systems Excellent clinical judgment and critical thinking skills Strong communication and patient education abilities Ability to work independently, prioritize tasks, and manage a flexible schedule Valid driver's license and reliable transportation What We Offer Competitive pay + benefits after 90 days Fully paid training (we invest in your success) Hybrid work model (approximately 50/50 in-office and remote) Structured ramp-up period to build your caseload Generous compensation per case management activity
