Remote Hemophilia of Georgia (HoG) is a nationally and internationally recognized nonprofit organization that provides programs and support services for people with hemophilia, von Willebrand Disease, and other bleeding disorders. Hemophilia of Georgia operates an annual summer camp program for children and teens with hemophilia and other inherited bleeding disorders. Camp is typically held in the summer (which may include the Fourth of July) and runs Saturday-Friday, and the date changes each year. Camp is an agency-wide, strategic priority; therefore, all exempt employees must participate. Assignments may include duties at camp, helping campers get to and from camp, and may involve overnight travel. The Director of Camp & Client Events will provide advance notice to exempt employees of where support is needed. PTO requests during the week of camp are generally not approved ; however, PTO requests for extenuating circumstances will be reviewed on a case-by-case basis by the Chief Community Engagement Officer. The Outreach Nurse provides education and assessment and assists with services coordination to HoG clients and their families. The Outreach Nurse functions as a member of the comprehensive care team at the hemophilia treatment center. The Outreach Nurse provides consultation and education to schools, hospitals, physicians' offices, and other community resources.

Qualifications:

Nursing degree from an accredited nursing program Registered Nurse licensed in the state of Georgia. Maintain license as required. A minimum of three years' experience providing clinical care Demonstrated clinical competence in infusion skills Self directed; ability to work independently; ability to multi-task Ability to prioritize work assignments and manage time efficiently Proven ability to work effectively in a team environment Ability to work without supervision, to use independent judgment, and to make appropriate decisions. Must have high level of interpersonal skills to handle sensitive and confidential situations. Position continually requires demonstrated poise, tact and diplomacy. Essential Functions:

Develop and maintain a sound knowledge base in all facets of hemophilia and other bleeding disorders, the nature of the disorders, treatment, complications and their treatments, client population, aspects of Hemophilia of Georgia, Georgia resources, etc. Analyze and review caseload of clients as assigned by the Director of Nursing and the Vice President of Programs. Determine client's level of eligibility for HoG programs and services as specified by HoG's tiering system and provide services as applicable. Visit client/family in home setting for assessment and education, including home therapy training. Document all encounters with clients, utilizing appropriate nursing documentation within the required time frame. Assist client in the preparation of a plan to address emergencies. Educate the client's medical and community support system (e.g., the emergency room, local physician, school, or daycare). Review each client's needs and care plan, with input from the social worker, as needed. Coordinate care, which may include crisis intervention, assistance with home care, in-depth counseling and support, providing letters, consults or referrals as indicated by individual needs. Refer to other resources as needed (e.g., pharmacy, hematologist, dentist, hemophilia treatment center, infectious disease physician, primary care physician, health department, CDC, support groups, information sources). Attend hemophilia clinics as directed to provide hemophilia education and information about HoG services to client and family including Telehealth clinics or home Telehealth care as directed Participate in pre-and post-clinic conferences and provide information about clients to other hemophilia team members. Provide follow-up care, including implementation of clinic care plan and follow-up referrals as needed. Provide additional clinical services in the home as ordered by the physician (e.g., infusions of blood clotting agents, port care). Consult regularly with treatment center nurses and social workers to coordinate client care. Meet or have telephone conferences regularly with social worker or team members to provide updates and coordinate client care. Educate people with bleeding disorders about the benefits of comprehensive care and HoG services to encourage participation and utilization. Provide opportunities for peer support between clients according to guidelines. Engage in activities designed to identify new clients (e.g., in-services, health fairs, outreach to physicians). Complete family pedigrees on each hemophilia client in order to identify potential carriers and/or people with bleeding disorders. Provide education about bleeding disorders and their complications (e.g., hepatitis, arthritis, inhibitors). Provide information on genetics and appropriate referrals for testing. Discuss appropriate physical activities. Assess clients for HoG safety program needs (e.g., helmets, Medic Alert®). Address with clients and their family members other issues of concern as identified by the client, consulting with the social worker when appropriate. Assist with pharmacy product recalls as required. Act as Camp Nurse at Camp Wannaklot for one week each summer. Attend client programs and activities, participating as directed. This may include overnight activities. Be successfully checked annually by the Director of Nursing for clinical skills, teaching skills and presentations. Adhere to professional standards and HoG Nursing Guidelines. Seek training sessions and workshops to keep current with changing medical technologies. Remain informed of research and other resources for client education and referral regarding issues of concern to people with bleeding disorders. Attend Home Therapy Committee meetings and assist with the home therapy/factor program as needed. Implement HoG's disease management program for the pharmacies' clients as directed by the Director of Nursing and the Vice President of Programs. Assist with Hemophilia Treatment Center(s) remote telehealth clinic(s) Assist with identifying clients to attend telehealth clinic Assist with standard operation and procedures of telehealth clinic(s)

Additional Responsibilities:

Participate on a team(s) at HoG—both programmatic and cross-departmental teams as assigned and/or appropriate. Contribute articles to HoG's newsletter. Assist in the orientation and training of new team members as requested by the Director of Nursing. Assist with general marketing strategies. Assist HoG in its ongoing programmatic efforts. Additional duties as assigned by the Senior Leadership or Leadership team member.
