Position Summary We are seeking an experienced and dependable Registered Nurse to provide skilled home health visits on a PRN, as-needed basis. The RN will assess patients, provide skilled nursing care, communicate with physicians and members of the interdisciplinary care team, and complete accurate and timely clinical documentation. The ideal candidate has at least one to two years of skilled home health experience and understands Medicare home health requirements, plans of care, physician orders, OASIS documentation, infection prevention, and patient-centered care. Key Responsibilities Perform skilled nursing visits in patients' homes Complete comprehensive nursing assessments Perform start-of-care, resumption-of-care, recertification, transfer, and discharge assessments when assigned and qualified Complete OASIS assessments accurately and within required timeframes Develop, implement, and update individualized plans of care Monitor patients' conditions and promptly report significant changes Provide medication reconciliation, medication education, and disease-management teaching Perform wound care, diabetic care, injections, catheter care, and other skilled nursing procedures within the RN's scope of practice Educate patients and caregivers about diagnoses, medications, treatments, safety, and emergency planning Obtain and implement physician or authorized practitioner orders Coordinate care with physicians, therapists, LPNs, home health aides, pharmacies, and other healthcare providers Supervise LPNs and home health aides when assigned Participate in case conferences and care coordination activities Complete all clinical documentation accurately and within agency timeframes Follow CMS Conditions of Participation, Ohio requirements, ACHC standards, HIPAA requirements, and agency policies Report incidents, medication concerns, infections, hospitalizations, and changes in condition promptly Required Qualifications Current, unrestricted Registered Nurse license in Ohio Minimum of one to two years of skilled home health nursing experience preferred Current CPR/BLS certification Strong clinical assessment and documentation skills Familiarity with OASIS and Medicare home health documentation Ability to work independently and manage a flexible visit schedule Reliable transportation and willingness to travel to patient homes throughout Central Ohio Valid driver's license and current automobile insurance Ability to use an electronic medical record system and mobile technology Ability to pass required background checks and pre-employment screenings Preferred Qualifications Experience in a Medicare-certified home health agency Experience with wound care, diabetes management, medication teaching, and chronic disease management Experience completing OASIS assessments and coordinating interdisciplinary care Schedule PRN/as needed Field-based patient visits Daytime, evening, weekend, and on-call availability may be offered based on agency and patient needs Visit assignments depend on patient census, location, and clinician availability This position does not guarantee a minimum number of weekly hours or visits Why Join SU Caring Hands?
