Massachusetts | Hybrid Remote $45.00 - $50.00/Hour Contract Through End of Year We are seeking Registered Nurses to support Medicare and Medicaid members throughout Massachusetts through community-based assessments and care coordination. This opportunity is ideal for nurses looking to transition from a demanding clinical setting into a role that offers flexibility, autonomy, and a strong work-life balance. Nurses work remotely from home when not conducting scheduled member visits, with a predictable weekday schedule and manageable daily workload. We are building teams throughout the entire state of Massachusetts and are looking for nurses in all regions. Wherever you live in Massachusetts, your member visits will be focused within your local territory. There is no expectation of traveling across the state. What You'll Do Conduct 1-2 scheduled member assessments per day Meet with members in their homes to complete needs-based assessments Identify medical, behavioral, and social support needs Document assessment findings and recommendations Ideal Backgrounds Home Health Visiting Nurse Association (VNA) Skilled Nursing Facility (SNF) Nursing Home Rehabilitation Behavioral Health Psychiatric Nursing Outpatient Clinics Community Health Hospice Care Management Case Management Schedule & Work Environment Monday through Friday, 9:00 AM - 5:00 PM (Remote when not visiting patients) -Optional weekend overtime shifts available, but not required Hybrid remote schedule Work from home between scheduled member visits Typical assessment times occur during morning and afternoon appointment blocks Assessments are scheduled locally within your area Nurses are assigned members close to where they live whenever possible Flexible and highly independent work environment What We're Looking For Active Massachusetts Registered Nurse (RN) license Associate Degree in Nursing or higher Minimum 1 year of outpatient nursing experience Experience in Home Health, Long-Term Care, Skilled Nursing, Rehabilitation, Behavioral Health, Hospice, Community Health, Care Management, or similar outpatient settings Strong assessment and communication skills Comfortable working independently Valid driver's license Reliable transportation and auto insurance Why This Opportunity Stands Out Hybrid remote work environment Significant work-from-home flexibility Approximately two assessments per day Local territory assignments based on where you live No hospital-based responsibilities Consistent weekday schedule Opportunity to build meaningful relationships with members Independence and autonomy throughout the workday Meaningful work supporting underserved populations Large hiring initiative with openings available across Massachusetts Compensation $45.00 - $50.00 per hour Exact compensation may vary based on skills, experience, education, and location.
