Job Summary We are seeking a compassionate and dedicated Hospice Nurse Case Manager to provide exceptional end-of-life care to patients in their homes and community settings. In this vital role, you will deliver comprehensive nursing services, coordinate patient care plans, and support families through difficult transitions. Your expertise will ensure patients experience comfort, dignity, and quality of life during their final stages. This position offers an opportunity to make a meaningful difference by combining clinical excellence with heartfelt compassion. Responsibilities Conduct thorough patient assessments, including vital signs, symptom management, and physiological evaluations to determine care needs. Develop and implement personalized care plans in collaboration with physicians, social workers, and family members. Administer medications and treatments aligned with hospice protocols while monitoring for adverse effects. Provide emotional support and education to patients and their families regarding disease progression, symptom control, and end-of-life choices. Coordinate discharge planning, ensuring smooth transitions between inpatient, outpatient, or post-acute care settings. Document all patient interactions accurately in medical records while maintaining HIPAA compliance. Engage in case management activities such as utilization management, diagnostic evaluation, and post-acute care planning to optimize patient outcomes. Qualifications Valid nursing license with current registration in the applicable state. Knowledge of infectious disease care, geriatrics, behavioral health, and medical-surgical nursing practices. Strong understanding of medical terminology, physiology, and diagnostic evaluation techniques. Proven ability to perform comprehensive patient assessments and discharge planning effectively. Excellent communication skills for patient service delivery and family education. Ability to manage complex cases involving post-acute care or managed care environments.
