We are seeking an experienced and compassionate Registered Nurse (RN) Clinical Case Manager to provide clinical oversight, care coordination, assessment, and case management services for individuals receiving home- and community-based services. Position Summary The RN Clinical Case Manager is responsible for: Assessing clients' medical, functional, psychosocial, and clinical needs Developing and monitoring individualized plans of care Coordinating healthcare and supportive services Monitoring changes in clients' health and functional status Collaborating with physicians, caregivers, providers, families, and other members of the interdisciplinary care team Helping clients safely maintain the greatest possible level of independence The ideal candidate is highly organized, clinically knowledgeable, compassionate, dependable, and comfortable working independently in community-based settings. Essential Duties & Responsibilities The RN Clinical Case Manager will: Conduct comprehensive nursing and clinical assessments of clients. Assess clients' medical, functional, psychosocial, environmental, and safety needs. Develop, implement, review, and update individualized plans of care. Monitor clients for changes in health status, functioning, medications, behaviors, or support needs. Conduct scheduled home, community, and client visits as required. Coordinate medical care, behavioral health services, personal support services, therapies, durable medical equipment, and other necessary resources. Communicate and collaborate with physicians, pharmacies, hospitals, caregivers, family members, and other healthcare professionals. Review medications and identify potential medication-related concerns within the RN scope of practice. Provide client and caregiver education regarding: Health conditions Medications Safety and prevention Care-plan goals Health maintenance Identify potential health and safety risks and initiate appropriate interventions, referrals, and follow-up. Assist with transitions of care following hospitalization, rehabilitation, emergency department visits, or changes in level of care. Monitor adherence to established plans of care and evaluate the effectiveness of services. Participate in interdisciplinary care conferences and case reviews. Advocate for clients and assist clients and families in navigating healthcare and community-based service systems. Maintain accurate and timely documentation, including: Nursing assessments Progress notes Plans of care Case-management documentation Follow-up documentation Other required clinical records Maintain client confidentiality and comply with HIPAA requirements. Recognize and appropriately report suspected abuse, neglect, exploitation, significant incidents, or other safety concerns. Follow applicable Pathway to Empowered Living LLC policies and state, federal, Medicaid, and program requirements. Perform additional clinical case-management duties within the RN scope of practice as assigned. Required Qualifications Applicants must meet the following requirements: Current, active, and unrestricted Registered Nurse (RN) license in the State of Georgia OR a qualifying multistate RN license that authorizes practice in Georgia Graduate of an accredited nursing program Strong clinical assessment and critical-thinking skills Excellent written and verbal communication skills Strong documentation, organization, and time-management skills Ability to work independently and collaboratively with an interdisciplinary care team Ability to effectively work with elderly adults and individuals with physical, developmental, behavioral, or other disabilities Ability to maintain professional boundaries and client confidentiality Ability to successfully complete required background checks, credential verification, and employment screenings Applicants who do not possess an active RN license that authorizes practice in Georgia will not be considered for this position.
