Remote documentation + Community/Home Visits About the Role As part of our maternal and infant health team, you will: Provide compassionate, non-skilled nursing care to mothers, infants, and families in home and community settings. Conduct comprehensive client assessments and help develop individualized, holistic care plans. Monitor client progress and adjust interventions to support healthy outcomes. Educate families on prenatal/postpartum care, infant health, parenting support, and self-care strategies. Collaborate with social workers, healthcare providers, and community resources for coordinated care. Maintain accurate, timely, and compliant medical documentation within the EMR. Follow all infection control, safety, and compliance standards.
Qualifications Required:
Valid Michigan RN License Minimum 1 year nursing experience Minimum 1 year maternal/infant health experience Current CPR/BLS certification Strong communication skills and family-centered approach Ability to work independently and make sound clinical decisions
Preferred:
