Job Description

Job Overview Shasta Family Health Care Services is a growing home health agency based in Anderson, California, seeking a compassionate and skilled Registered Nurse (RN) for a per diem position. In this role, you will provide high-quality, patient-centered care in the home setting, supporting patients in recovery, chronic disease management, and maintaining independence. This position is ideal for an RN who is comfortable working independently, values flexibility, and enjoys building meaningful patient relationships.

Per Diem (PRN)
Pay:
$55-$60 per hour, depending on home health experience, plus additional per-task rates (e.g., OASIS assessments, SOC/ROC visits)
Travel:
Local travel required Responsibilities Perform skilled nursing visits in patients' homes in accordance with physician orders and plan of care Conduct comprehensive assessments, including vital signs, physical exams, and health history Complete OASIS assessments (SOC, ROC, recertification, and discharge); prior OASIS experience is preferred For nurses without OASIS experience: must be willing to learn and complete OASIS documentation with structured agency guidance and support Develop, implement, and update individualized care plans Provide wound care, including dressing changes and ostomy care Perform medication reconciliation, administration, and patient education Educate patients and caregivers on disease management, safety, and self-care Document all care accurately and timely in Axxess EMR Coordinate care with physicians, therapists, and interdisciplinary team members Participate in case management and discharge planning Maintain compliance with HIPAA, infection control, and agency policies Qualifications Active California RN license in good standing Current BLS certification Minimum 1 year of nursing experience required Home health experience and OASIS experience strongly preferred Experience in med-surg, ICU, or ER is a plus, but not required Strong clinical assessment and documentation skills Ability to work independently and manage time effectively in a home setting Comfortable working with geriatric and medically complex patients
Pay:
$55.00 - $60.00 per hour
