Registered Nurse (RN) - Bilingual Preferred (Hybrid, I/DD Waiver Program)

Job Overview:

We are looking for a compassionate and experienced Registered Nurse (RN) to join our team in supporting individuals enrolled in I/DD Waiver programs. This hybrid position requires a minimum of one-two days per week in the field conducting Annual Nursing Assessments for individuals receiving HHSC funding, with the remainder of the work completed remotely from home.

Duties:

Conduct comprehensive annual nursing assessments in compliance with HHSC and waiver program guidelines. Travel to client homes, group homes, or community settings across the Houston area. Accurately document findings and submit timely reports. Collaborate with interdisciplinary teams to ensure quality care and service coordination. Maintain current knowledge of HHSC standards and I/DD Waiver program requirements.

Qualifications:

Active RN license in the state of Texas. Minimum of 2 years of nursing experience, preferably in community health, home health, or I/DD services. Strong clinical assessment, communication, and documentation skills. Reliable transportation and willingness to travel locally.

Preferred Experience:

Familiarity with HHSC-funded I/DD Waiver programs (e.g., HCS, TxHmL) Experience working with individuals with intellectual and developmental disabilities. Bilingual in English and Spanish strongly preferred.
