A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Job Description
WHAT YOU WILL DO
- Conduct comprehensive physical and psychosocial assessments in accordance with EDWP guidelines and regulatory standards
- Complete care plans and assessments based on services requested
- Submit documentation within required Standards of Promptness
- Educate clients and caregivers regarding rights, responsibilities, and available services
- Verify Medicaid eligibility using established
DFCS/MAO/PMAO
protocols- Review referral documents and relevant medical and social history
- Organize travel routes and daily workflow to ensure timely completion of visits and documentation
- Participate in staff meetings, training, and care team case conferences
- Perform other related duties as needed
WHAT WE ARE LOOKING FOR
- Current Georgia nursing license in good standing
- Minimum 2 years of nursing experience, preferably in geriatric nursing, community health, long-term care, or chronic disease management
- Graduate of an approved nursing program
- Current CPR and First Aid certification
- Reliable transportation, valid driver's license, and proof of insurance
- Ability to pass criminal background check and drug screening
- Self-directed, organized, and comfortable managing your own daily schedule and territory
- Genuine interest in supporting older adults and individuals with disabilities in their home environments
TERRITORY & SCHEDULE
Work primarily remote with scheduled in-home visits across Candler County and surrounding areas (approximately 1.0 - 1.5 hour radius, all within Georgia). This position is shared with another nurse to balance travel and caseloads. You set your own schedule within business hours while meeting visit and documentation timelines. Monday through Friday.WHY THIS ROLE
