Job Description

Overview As a Hospice Nurse/RN Case Manager Full-Time, you will play a vital role in providing compassionate care to patients facing life-limiting illnesses. Your expertise will guide families through challenging times, ensuring comfort and dignity for those in your care. You will collaborate with an interdisciplinary team to deliver holistic support, focusing on pain management, emotional well-being, and quality of life. Join us in making a meaningful difference in the lives of patients and their families! RN Case manager experience preferred. What you'll do Deliver comprehensive nursing care to hospice patients in various settings including homes, nursing facilities, and hospitals. Conduct thorough patient assessments to identify needs and develop individualized care plans. Administer medications and treatments as prescribed, ensuring adherence to safety protocols and best practices. Provide emotional support and education to patients and families regarding disease processes, treatment options, and end-of-life care. Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care effectively. Monitor vital signs and manage symptoms such as pain and discomfort using advanced techniques including IV infusion and acute pain management strategies. Maintain accurate medical documentation in compliance with HIPAA regulations and organizational standards. Participate in ongoing training and development to enhance skills in hospice & palliative medicine. Basic qualifications Valid nursing license with experience in hospice or palliative care settings. Preferred qualifications Proficiency with electronic medical records (EMR) systems such as Epic or Cerner. Experience with critical care transport or within a Level I or II trauma center. Knowledge of
ICD-9/ICD-10
coding for accurate medical documentation. Familiarity with geriatric care principles, including memory care and working with individuals with developmental disabilities. Skills in performing procedures such as suturing, venipuncture, catheterization, and administering injections.
