Hospice East Bay - Nonprofit Hospice (Serving the community since 1977) Overview Hospice East Bay is a well-established nonprofit hospice that never turns a patient away due to inability to pay. We offer comprehensive services including Pet Therapy, and Bereavement Care, and we are proud to be certified as a Best Place to Work for the past six years. We are currently seeking compassionate and skilled Admission Nurses to join our team. Position Summary The Admission Nurse is the first point of contact for patients and families entering hospice care. This role includes completing assessments, determining eligibility, providing education, and ensuring a smooth, supportive transition into services. Work is primarily in patient homes, facilities, and healthcare settings, with travel required within our service area. Key Responsibilities Conduct comprehensive patient assessments and evaluate hospice eligibility. Educate patients and families on hospice philosophy, services, and symptom management. Complete admission paperwork, consents, and documentation accurately and promptly. Coordinate with physicians, referral sources, and interdisciplinary team members. Develop care plans based on patient and family goals. Provide emotional support and address questions during the admission process. Maintain regulatory compliance and up-to-date documentation in the EMR. Requirements Active California RN license. Valid California driver's license, insurance, and reliable transportation. Minimum 2 years of nursing experience (hospice, palliative care, or home health preferred). Strong clinical assessment, communication, and critical-thinking skills. Knowledge of hospice philosophy, Medicare/Medi-Cal, and insurance regulations. Ability to work independently and as part of an interdisciplinary team. Ability to lift/transfer patients and perform necessary physical tasks. CHPN certification preferred, not required. Benefits 403b Retirement Account and generous company match.
