About Home Heart Florida We're a pediatric home health agency helping children with complex medical needs thrive at home. The Role As our SOC Nurse, you'll complete initial home evaluations for pediatric patients, establish care plans, and set the tone for a safe, high-quality home health experience. You'll be a key touchpoint for families at the most important stage of their care journey. What You'll Do Conduct comprehensive start of care assessments in the home setting Develop and initiate individualized care plans based on physician orders Educate family caregivers on care needs, equipment (vents, g-tubes, pumps), medications, and safety Complete thorough, accurate clinical documentation Communicate findings and care plans to the clinical team and coordinating nurseCore Requirements Please only apply if you meet all of the following: Active Florida RN license + BLS certification 1 year + experience completing Start of Care visits in home health Strong clinical documentation and EMR experience Valid Drive License & Reliable transportation and willingness to travel to patient homes Comfort working with medically complex pediatric patients Preferences Bilingual English / Spanish (especially for Miami-Dade and Broward) Experience with Statewise EMR Pediatric home health or private duty nursing experience Why Nurses Like Working With Us Competitive per-visit compensation Flexible schedule (per diem or part time) Opportunity to grow into full-time leadership roles Opportunity to develop advanced pediatric home health assessment skills Clinical mentorship and training to strengthen SOC documentation and care planning A team that genuinely supports each other — and keeps cafecito stocked ☕ Help us give kids the best possible start to their care at home.
