Responsibilities Perform comprehensive patient assessments including vital signs, physical examinations, and medical history reviews to determine appropriate care strategies. Administer medications and treatments such as IV infusions, injections, suturing, and wound care following strict aseptic techniques. Utilize electronic health record (EHR) systems like Epic or Cerner to document patient information accurately and efficiently. Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of complex care needs including dialysis, cardiac catheterization, trauma medicine, and hospice services. Conduct specialized procedures such as catheterization, tube feeding management, ostomy care, and neonatal resuscitation procedures when required. Provide triage assessments in urgent care or emergency room settings to prioritize patient needs based on acuity levels. Educate patients and families on health coaching topics including geriatrics, memory care, behavioral health, pain management, and disease prevention strategies aligned with NCQA standards. Skills Proven experience working with diverse patient populations across settings such as

ICU, PICU

(Pediatric Intensive Care Unit), NICU (Neonatal Intensive Care Unit), or trauma centers. Proficiency with EMR/EHR systems including Athenahealth or eClinicalWorks for medical documentation and coding (ICD-10/ICD-9). Strong knowledge of physiology, anatomy, pharmacology, and medical terminology essential for accurate patient assessment and diagnosis. Skilled in advanced clinical procedures such as venipuncture (phlebotomy), IV insertion/infusion, ventilator management, and sterile processing techniques. Familiarity with infectious disease control protocols and infection prevention practices. Experience providing specialized care in areas like geriatrics (memory/dementia/dementia care), behavioral health, occupational health or home & community care environments. Ability to perform diagnostic evaluations including laboratory specimen collection/processing or sonography when applicable. Excellent communication skills for educating patients about treatment plans while ensuring compliance with HIPAA regulations. Capable of managing acute pain cases and participating in trauma or emergency medicine response efforts with a focus on patient safety and comfort.
