Job Description
Job Summary:
Coordinates systems and procedures with the rehab team, facilitator, and mentor to cultivate a safe, efficient, and effective environment of care. The RN is responsible for designing, implementing, and evaluating patient care by coordinating, delegating, and supervising the care provided by the healthcare team. Specifically, guides staff or provides direct patient care, including evaluation and physician prescribed exercise with an emphasis on development of individualized exercise programs based on initial assessment/results.Provides risk factor modification, education and counseling in order to limit adverse physiologic and psychological effects of cardiac, pulmonary or vascular illness, provides secondary prevention and enhances optimal wellness.
