Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members Travel to patient residences across areas including Providence, Woonsocket, Scituate, and Cranston Review prior and current medical history, conditions, and medication records Educate patients on their diagnoses, treatment plans, and prescribed medications Identify care gaps and provide recommendations for ensuring continuity and quality of care No development of treatment plans or prescription of medications required

Qualifications:

Active Nurse Practitioner License in Rhode Island Current CPR certification (BLS) Board Certification by AANP or

ANCC MSN

from an accredited school of nursing At least 6 months of current Nurse Practitioner experience preferred This role offers compensation of $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
