_PARAGRAPH JOB_DESCRIPTION.SHARE.HTML VNA of Care New England Warwick, Rhode Island 8034 Medical Nursing Full-Time Other Union Other 40.00 Job Description Job Summary Under the direction of the Clinical Manager/Director, the After-Hours Visiting RN provides home health and hospice care during scheduled after-hours coverage. Responsibilities include scheduled and PRN visits, admissions, and hospice death visits. The RN must be flexible with assignments, communicate concerns to leadership, and perform all duties in accordance with agency policies, professional standards, and applicable regulatory requirements. The RN must be able to work independently, exercise sound clinical judgment, and provide timely documentation and communication. Requirements Equivalent education required for RI licensure required, Bachelor's Degree in relevant discipline strongly preferred. Hospice and OASIS certification preferred but not required. Obtaining Hospice and OASIS certification may be required during employment. Valid CPR certification required. Valid RI Board license (for specific discipline RN), valid driver's license and automobile liability insurance are required. A minimum of one (1) year progressive clinical experience required. Previous experience in an acute care or Home Health care environment setting preferred. Hospice experience preferred. Ability to read and interpret documents such as medical records, medical reports, etc. Ability to write reports, business correspondence and procedure manuals. Must be able to speak, read, and write English with excellent oral and written communication skills required. Skilled in using standard office software (e.g., email, word processing, scheduling, and secure messaging platforms) to communicate with interdisciplinary team members, physicians, and on-call leadership. Ability to quickly learn new clinical software systems, accurately enter and retrieve patient data, troubleshoot basic technical issues, and maintain compliance with HIPAA and organizational policies related to electronic documentation and data security. Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions furnished in written, oral, diagram or schedule form. Must be able to deal with several abstract and concrete variables.
